Welcome PharmaCare Employees!

If you are a PharmaCare/ProCare employee, information about how this integration will affect your benefits, your paychecks, and your access to the HR tools and resources you need to make smart choices was distributed on May 7 at your worksite. Be sure you receive the following:

• The 2008 PharmaCare Benefits Guide

• The 2008 PharmaCare Highlights Brochure

• Your Personal Report, which outlines your benefits choices and premiums

Your benefits enrollment deadline is May 21 at 5:30 p.m. CT. This is not a passive enrollment, which means if you do not enroll by the deadline date, you will have no medical, dental, FSA, or vision benefits after May 31.


New Hire Enrollment Checklist

In order to make the best enrollment decisions possible, it is important that you are completely prepared. Prior to beginning your online enrollment process, you should “map out” your successful enrollment by following the strategies below:

Review Your Kit–Thoroughly review the Enrollment Kit and the Personal Report that you received at your worksite.  “Custom-built” just for you, the Personal Report outlines your benefit choices and indicates your biweekly premium costs.

•Vendor links, Member Services phone numbers, forms, and resources

Consider Your Benefit Needs–Choosing the healthcare options that meet the unique needs of you and your family can be challenging. Keep in mind that your benefits don’t just protect you from financial hardship in the case of injury or illness—they may also provide you with preventative services like annual physicals and screenings. There are a few things to consider when choosing the healthcare options that work best for you:

• Do you need coverage for yourself only, or do you also want to cover any eligible dependents?

• Does someone in your family have any special medical conditions that require regular visits to doctors or specialists?

• What dental services will your family need?

• Do you, your spouse, or children need braces or other special dental services?

• Will you use pre-tax dollars from your healthcare Flexible Spending Account (FSA) to help offset the costs of your deductibles and other out-of-pocket expenses?

• Do you, your spouse, or your children have coverage options under other healthcare plans?

• Do those plans coordinate with the Caremark offerings?

• Do you have sufficient Life Insurance?

Have Your Employee Self Service User ID and Password Available–Your User ID is found on the top of your Personal Report, and your initial Password is the letter P, followed by the last four digits of your social security number .  If you have lost or forgotten your User ID or Password, contact FrontLine Services for assistance. FrontLine Services can be reached 365/24/7 via e-mail at FrontLine.Services@Caremark.com, or by phone toll-free at 1-800-773-4003.

Be Prepared With Data–Have the names, birth dates, and social security numbers of all the dependents and/or beneficiaries you are planning to enroll and/or name as beneficiary. Having this information readily available ensures a quicker and more accurate enrollment.

Look for Your Confirmation StatementThe key to your successful enrollment is accuracy! Your Confirmation Statement is the complete and final record of your benefit elections. Protect yourself and ensure it is correct by thoroughly reviewing this statement, which will arrive at your home about seven days after you enroll.  If you do not receive your Confirmation Statement within 7-10 days of your enrollment, immediately contact the Caremark Benefits Center (CBC) toll-free at 1-888-769-1717. Any changes must be reported to the CBC within 14 days of the date printed on your Confirmation Statement.  Changes after this date will not be accepted.
