While the basic plan design information for each medical option is summarized in 2009 Benefits Guide, there is so much more to consider when choosing a medical plan. Choosing the correct plan for you and your family can be an overwhelming process—so let the tools below make it easier for you! The four components of the Health Plan Decision Toolkit are shown below. Use one or all four to make smart choices.
eZChoice: A Shortcut to Better Benefits DecisionseZChoice makes choosing your medical plan faster and easier. Just answer a few questions and eZChoice responds by: • Suggesting a specific medical plan option for you and your dependents |

| •Showing you how much each suggested medical plan option will cost from your paycheck |
| •Advising if you will save money by enrolling in the Health Care FSA |
Medical Expense Estimator: Find Your Financial “Fit”The Medical Expense Estimator tool suggests the option that is your best financial fit, based on the way you and your family use healthcare. Use the Medical Expense Estimator to compare your projected total share of annual medical/prescription expenses (paycheck contributions and out-of-pocket expenses) for each available option. You’ll also have access to the Spousal Plan Comparison feature, which lets you project and compare costs for a plan offered by your spouse’s employer with the CVS Caremark medical plan options. The option with your lowest projected out-of-pocket costs, is usually your best deal.
Health Plan Comparison Tool: Avoid SurprisesUse the Health Plan Comparison Tool to avoid coverage surprises. It helps you create a chart that compares up to three medical plan options. It also provides annual paycheck costs, prescription drug costs, and your medical carrier’s website and contact information.
Health Care FSA Estimator: Estimate Your Tax SavingsUse the Health Care FSA Estimator to add up the eligible health care expenses that your medical/prescription, dental, or vision plans don’t pay for, and find out how much you can save in taxes in the upcoming year. Click here to access the Health Plan Decision Toolkit. Remember, you’ll need the GroupCode, which is listed on the top right of your Personal Report, in order to access the correct pricing.
